Case Manager

Latin American Youth Center's (LAYC) mission is to empower youth to achieve a successful transition to adulthood, through comprehensive and innovative programs that address youths' social, academic, and career needs. To achieve this mission, LAYC provides services and opportunities to approximately 4,000 individuals annually to support academic achievement, promote healthy behaviors, and guide youth toward successful adulthood.

ESSENTIAL RESPONSIBILITIES

Coordinates case management services to children and their families.
Coordinates and assists with the safe and timely release of children to their sponsors.
Conducts intakes and assessments on children and families to ensure well-being.
Works on a multidisciplinary team to ensure delivery of wrap around services.
Serves as a liaison to children and stakeholders.
Documents all contacts with and/or on behalf of clients within 48 hours of contact.
Complete other duties as assigned.
