Case Manager

The Tree Grant Case Manager promotes and supports individuals who are at risk of developing or have mental health and/or substance abuse issues ,and works with individuals and families with at least one member with a substance use and/or mental health disorder to establish appropriate linkages; referrals; coordination of care; and provides regular follow-up on needed services and supports. Overall services include assessing, monitoring, linking to services, supportive level counseling, education, and advocacy. This is a grant-based position that will end with the ending of the grant.
