Position Summary
The Mental Health Advocate provides supportive counseling and case management services to adult behavioral health clients under the direction of a licensed professional. Supportive counseling may include symptom management and skills training using behavioral principles and a structured session approach. Case management includes care coordination, resource identification and development, advocacy, service monitoring and follow-up, collateral supports.
Responsibilities
Serves as a consumer advocate in relation to the mental health system, community agencies, and the consumer’s support system, as needed.
Provides person-centered supportive counseling, assessment of needs/risk, skills training, advocacy and case management for clients
Assists in developing problem-focused, goal-oriented treatment plans which address the specific needs of the consumer and specific anticipated clinical outcomes.
Provides comprehensive community support activities to assigned caseload, including assistance in identifying and utilizing community resources, liaison with other agencies and organizations, referral to appropriate community programs, completion of all necessary reports.
Maintains consumer records in keeping with Agency, County, and State guidelines.
Coordinates discharge planning, including referral to appropriate agencies for ongoing needs.
