Care Coordinator I, Bilingual (Spanish)

As a Care Coordinator I, you will provide case management, mental health and substance use services, crisis intervention, and linkage to community resources for child clients with a history of persistent mental illness or substance use. You will ensure clients maximize the benefits of all interventions in an integrated health care setting and maintain a safe, nurturing environment that fosters hope, dignity, and recovery.
