Job Summary (Including, but not limited to)
Participate in morning meetings, staffing and other assigned meetings with participants, family members and collateral agencies to support assigned participants in expressing their perspective.
Partners with NT19 Connective Care Coordinator to provide coverage support when needed or assigned.
Work with an assigned caseload of NT19 participants to develop and achieve specific ISRP goals.
Assistance in identifying goals in the areas of living, learning, working, wellness, and self-advocacy.
Assistance in implementing ISRP goals by providing transportation, support in setting appointments, debriefing successes, and next steps, coaching through periods of frustration, and identifying natural and family-based supports and community resources that enhance self-efficacy and self-sufficiency.
Meet with participants in the hospital/jail/ASH.
Conduct hospital discharge follow-up.
Participate in outreach, engagement, and re-engagement with a focus on community-based re-engagement.
