Position Summary

This is a work from flexible position with 50-75% travel to meet members face to face.

This role provides comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues.

This includes:

-Determines medical necessity/ appropriateness

-Facilitates optimal outcomes

-Identifies and follow through with continuous quality/ compliance opportunities. May also include identification of aberrance's and initiation of corrective action

-Educates/ empowers customers to ensure compliance, satisfaction and promote patient advocacy

-Optimize total costs Implementation and evaluation of policy based on usage and program directives

-Educate/empower colleagues at all levels to enable decision making at most appropriate level

Required Qualifications

-Minimum 2 years of experience in behavioral health, social services or human services field.
-Minimum 2 years of case Management experience.
-Willing and able to travel using your own vehicle up to 50- 75% of the time to meet members face to face in Northside of Chicago city and surrounding areas.

-Reliable Transportation required, eligible for mileage reimbursement as per company policy.

Preferred Qualifications
– Discharge planning experience preferred.
– Managed Care experience preferred.
– Microsoft Office experience preferred.


Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services required (psychology, social work, marriage and family therapy, counseling)

Pay Range

The typical pay range for this role is:

$19.52 – $42.07

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
